Biopsy for a lung mass

Short answer
A biopsy of a lung mass collects tissue to identify its cause when imaging cannot provide a definite diagnosis.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Finding
- A lung mass can have cancerous or non-cancerous causes.
- Method
- Imaging and anatomy determine how tissue is reached.
- Follow-up
- The pathology result must be considered with the scans and clinical history.
A clear overview
Not every lung mass is cancer. The sample can distinguish malignant cells from infection, inflammation and other changes, allowing care to be based on what the tissue actually shows.
How the sample is chosen
The mass's position, size and relationship to airways and blood vessels guide the approach. A clinician may pass a needle through the chest under imaging guidance, use a bronchoscope through the mouth or nose, sample an accessible lymph node, or obtain tissue surgically.
A pathologist studies the cells and surrounding tissue. Special stains may help establish where abnormal cells originated. If cancer is found, further testing can sometimes identify biological features relevant to therapy. An inconclusive result does not establish that the mass is harmless; it may mean the target was missed or the material was insufficient.
Before and after sampling
Tell the biopsy team about anticoagulants, antiplatelet medicines, supplements, allergies, pregnancy and lung or heart conditions. Use only the medicine and fasting instructions supplied for your planned technique. If sedation is expected, organise a responsible adult to take you home and follow the restrictions given at discharge.
Rest as directed and protect the wound dressing. Note whether breathlessness, chest discomfort or bleeding is improving. Keep the follow-up appointment even if you feel well, because the result and its limitations need to be interpreted alongside the scans.
Symptoms that need attention
Call emergency services for sudden or increasing difficulty breathing, intense chest pain, fainting, blue or grey colouring around the mouth, or substantial coughing of blood after the biopsy. Air leakage around the lung and bleeding require prompt assessment.
Speak with the care team without delay if you develop fever, chills, spreading redness, fluid from the wound, worsening pain, or persistent blood-streaked sputum beyond the pattern described in your discharge advice.
Useful questions before consenting
Confirm what the scan shows, why biopsy is preferred over surveillance or another test, and which route offers the clearest sample. Ask about anaesthesia, expected recovery, warning signs, when pathology should be available, and whether another sample would be needed if the report is non-diagnostic.
Questions people ask
Why can a lung mass biopsy be inconclusive?
The needle or instrument may collect too little material, miss the relevant part of the mass, or retrieve tissue altered by inflammation or damage. The team may advise repeat sampling, a different route or imaging follow-up.
Will I be awake during the biopsy?
That depends on the technique. Local anaesthetic is commonly used for a needle biopsy, while sedation or general anaesthesia may be used for bronchoscopic or surgical procedures. Your team should explain the plan beforehand.
