What should you know about bilirubin blood test?

Bilirubin forms when the body clears old red blood cells. The liver changes it and sends it into bile for removal through the bowel. The test may report total bilirubin and separate forms, helping a clinician consider whether a finding relates to increased breakdown, liver-cell irritation or reduced bile flow. A result is not a diagnosis by itself. Symptoms such as yellow eyes, dark urine, pale stools, itching, tiredness, fever or abdominal pain, along with other liver tests, provide the context. Anaemia, infections such as Brucellosis, Dengue Fever or Glandular Fever, and conditions affecting the liver or gut may be considered when appropriate.

Ask the laboratory whether fasting is needed and tell the clinician about medicines, supplements, alcohol use and recent illness. The sample is usually taken from a vein in the arm. If the result is outside the expected range, follow-up may include repeat blood work, examination, urine testing or an ultrasound. Management depends on the cause, which may include liver or bile-duct problems, increased red-cell breakdown, or an inherited tendency to have mildly raised bilirubin.

Arrange prompt medical review for new jaundice, dark urine, pale stools, persistent itching, unexplained fever or abdominal pain. Seek urgent help for severe pain, confusion, faintness, repeated vomiting, bleeding or rapidly worsening yellowing. Newborn jaundice needs the timing and severity assessed by a clinician.
