What bilious vomiting can indicate

Bile can appear after the stomach has emptied, particularly during repeated vomiting. It may occur with Gastroenteritis, Food Poisoning or Rotavirus Infection. Gallstones, Pancreatitis, Migraine and other illnesses can also cause vomiting. In a baby or child, green vomit is especially concerning because it can signal a bowel blockage and needs urgent assessment. Vomiting colour alone cannot identify the cause; clinicians consider pain, swelling, bowel movements, fever, headache, pregnancy, medicines and recent surgery.

What to do now

Stop solid food briefly if drinking triggers vomiting, then take small, frequent sips of water or an oral rehydration drink. Avoid alcohol and large amounts at once. Once fluids stay down, try gentle foods and return to usual meals gradually. Do not take anti-vomiting medicine unless a clinician or pharmacist says it is suitable, especially for a child. Keep a note of the colour, episodes, pain and urine output for the assessment.

When to seek care

Seek urgent care for green vomit in a child, severe or worsening abdominal pain, a swollen abdomen, inability to pass stool or gas, blood or coffee-ground material, faintness, confusion, severe headache with neck stiffness, or signs of dehydration. Adults should seek prompt medical advice when bilious vomiting repeats or fluids cannot be kept down.
