Bile acid itching: causes, assessment and relief

The term usually refers to itch associated with cholestasis, when bile does not move normally through the liver and bile ducts. The sensation may be widespread and can be intense on the palms or soles. Scratching can leave marks, yet a primary rash may be absent before scratching begins.

Itching alone cannot confirm that bile flow is impaired. Dry skin, eczema, contact reactions, hives, scabies and medicines can also cause pruritus. A clinician looks at the skin before focusing on liver or bile-duct causes and asks about yellow eyes or skin, urine colour, stool colour, abdominal symptoms, medicines and pregnancy.

Blood tests can assess liver and bile-flow patterns when the history suggests cholestasis. Further investigation depends on the findings and may include imaging to look for blockage. Treatment targets the underlying cause as well as the itch, so identifying the mechanism matters more than the label alone.

Book a medical review if the itch is persistent, widespread, interrupts sleep, or occurs without an obvious rash. Bring a list of medicines and supplements, and mention any recent changes. Do not stop a prescribed medicine abruptly unless the prescriber advises it.

While awaiting assessment, use lukewarm rather than hot water, apply an unperfumed moisturiser, wear loose breathable clothing and keep nails short. Cooling the skin can reduce the urge to scratch. Antihistamines do not address every cholestatic itch mechanism, so ask a clinician or pharmacist before relying on them.

Seek prompt medical care when itching comes with yellowing of the eyes or skin, very dark urine, unusually pale stools, abdominal pain, vomiting, fever, confusion or marked drowsiness. During pregnancy, new generalised itching—particularly on the palms or soles—should be reported promptly to the maternity team because pregnancy-related cholestasis requires specific assessment.

Ask whether the skin pattern points to a dermatological cause, which blood tests are appropriate, whether any medicine could contribute, and what treatment suits the suspected mechanism. If tests show impaired bile flow, clarify what may be obstructing or affecting it and how follow-up will be arranged.
