What should you know about bilateral hand swelling causes?

The timing and accompanying features matter. Puffiness after heat exposure, prolonged inactivity, or heavy hand use has a different pattern from persistent joint swelling with morning stiffness. Medicines and conditions affecting fluid balance can also cause swelling, so a new or unexplained change merits review.

A clinician will look at whether the swelling is mainly in the joints, fingers, or whole hands and whether pressing the skin leaves an indentation. Pain, warmth, redness, stiffness, rash, numbness, and swelling elsewhere provide further direction.

Rheumatoid arthritis commonly presents with pain, swelling, and stiffness in small joints on both sides, although bilateral hand swelling does not by itself establish that diagnosis. A distal radius fracture more often follows wrist trauma and affects the injured side; simultaneous injuries can involve both wrists. Examination and the history determine whether blood tests or imaging are appropriate.

Remove rings promptly before swelling increases. Rest the hands in a comfortable raised position, keep fingers moving gently if movement is not restricted by injury, and avoid tight bands. Record when swelling appears, how long it lasts, any new medicines, and whether the feet or face are also affected.

Seek a routine appointment for persistent, recurrent, or unexplained swelling, particularly when joint stiffness or reduced hand function is present. Do not stop a prescribed medicine without guidance; a prescriber can check whether it may be contributing and discuss alternatives.

Get urgent help for sudden swelling with breathing difficulty, wheezing, faintness, or swelling of the lips or tongue. A hand that is very painful, hot, rapidly reddening, blue, pale, cold, or losing sensation also requires prompt assessment. Emergency evaluation is warranted after major trauma, when rings cannot be removed from a tightening finger, or when swelling occurs with severe breathlessness or chest discomfort.

Ask whether swelling is centred in the joints or soft tissues, whether any medicine could contribute, and which associated symptoms would change the urgency. Bring a medicine list and, if the swelling varies, photographs showing the hands at their most swollen.
