A very large blackhead on the nose

A blackhead forms when oil and dead skin collect in an open follicle and the exposed material darkens on contact with air. Its colour is not trapped dirt. A single enlarged pore can also hold a compact plug, while a dark mark without a visible pore may have another cause.

Comedonal acne commonly presents with multiple blackheads and flesh-coloured bumps, often around the nose, forehead or chin. Inflammation can add red or tender spots. A dermatologist can use the surface pattern, pore opening and surrounding skin to distinguish a comedone from a cyst, dilated pore or pigmented lesion.

Size alone does not establish a diagnosis. A mark that changes colour or shape, bleeds without being picked, repeatedly crusts, or lacks a clear pore should be examined rather than treated as acne at home.

Cleanse gently and avoid digging with nails, needles or extraction tools. Forceful squeezing can drive inflammation deeper, tear the follicle and leave a dark mark or scar. Non-comedogenic moisturiser can reduce irritation from acne care.

For recurring blackheads, a clinician or pharmacist can help select a pore-clearing topical treatment. Topical retinoids can reduce new comedones but may irritate skin and require specific advice during pregnancy or when trying to conceive. A trained professional may remove a suitable plug with sterile technique; widespread or severe acne needs a broader treatment plan.

Arrange a skin examination if the spot is new and enlarging, looks unlike your other marks, changes colour or border, bleeds, ulcerates or fails to heal. Seek prompt care for rapidly increasing redness, warmth, swelling, pus, severe tenderness or fever after squeezing, as these can signal infection.

Ask whether the opening is truly a comedone, whether extraction is appropriate, and which topical product fits your skin. Mention previous acne treatments, irritation, pregnancy plans and any change in the mark's appearance so the clinician can choose a safe approach.
