A swollen or enlarged stomach after miscarriage

The abdomen may not immediately look or feel as it did before pregnancy. Hormonal changes can affect digestion, reduced activity and some medicines can contribute to constipation, and the uterus takes time to reduce in size. Tenderness or swelling can also occur after medical or surgical management. The pattern matters: general gassiness differs from a firm, increasingly painful, or one-sided swelling.

Ongoing pregnancy tissue, infection, urinary problems, bowel problems, or bleeding within the abdomen can also cause symptoms, although appearance alone cannot identify the cause. A clinician may review bleeding, pain, temperature, bowel and bladder function, pregnancy testing, and previous treatment. Examination and ultrasound can be used when the recovery pattern is unclear.

Note when the swelling began, whether it changes after eating or opening your bowels, and whether pain, bleeding, discharge, nausea, fever, or urinary symptoms occur with it. Gentle movement, regular fluids, and fibre-containing foods may help mild constipation if you can eat and drink normally. Check with a pharmacist or clinician before using a laxative, especially after a procedure or when taking other medicines.

Contact your maternity or gynaecology service if the abdomen remains enlarged, becomes more swollen, or recovery is not progressing as you were advised. They can determine whether follow-up testing or a transvaginal ultrasound is appropriate and discuss further medical or surgical management only if the findings support it.

Seek urgent medical help for severe or worsening abdominal pain, a hard or rapidly enlarging abdomen, fainting, marked dizziness, shoulder-tip pain, difficulty breathing, heavy bleeding, fever, chills, foul-smelling discharge, persistent vomiting, or inability to pass urine, stool, or gas. These symptoms can signal bleeding, infection, or another complication and should not be managed as ordinary bloating.

Ask whether the swelling fits your stage of recovery, whether the uterus and bleeding pattern are as expected, and whether examination, pregnancy hormone testing, or ultrasound would clarify the cause. Confirm which pain relief and constipation remedies are suitable, as well as whom to contact if symptoms change outside clinic hours.
