What a big red lump on the neck may mean

Possible sources include an infected skin cyst, boil, abscess, irritated lymph node, or inflammation near a salivary gland. Redness provides a clue but cannot identify the exact tissue involved.

Warmth, tenderness, softening in the centre, or drainage can occur when infection and pus collect under the skin. A deeper node can become tender during an infection in the throat, teeth, ear, scalp, or nearby skin. Swelling that worsens around meals can point toward a salivary gland problem.

Head and neck cancer, lymphoma, and thyroid cancer can present with a neck lump, although overlying redness is not enough to show that cancer is present. A clinician considers how long the mass has been there, whether it is mobile, and whether there are throat, thyroid, skin, or general symptoms.

Keep the skin clean, avoid tight collars, and do not squeeze or puncture the swelling. A clean warm compress may ease discomfort, but it should not delay assessment when the redness is spreading or the lump is enlarging.

Mark the edge of visible redness or take a dated photograph so you can describe change. Note fever, drainage, dental pain, sore throat, recent shaving, skin injury, trouble turning your head, and medicines that affect immunity. These details help distinguish a surface infection from a deeper neck problem.

Call emergency services for difficulty breathing, noisy breathing, inability to swallow saliva, tongue or facial swelling, confusion, fainting, or a rapidly expanding neck mass. Deep neck swelling can threaten the airway even when the skin opening looks small.

Seek same-day medical advice for spreading redness, fever, escalating pain, pus, red streaking, marked stiffness, or feeling very unwell. Prompt assessment is also warranted when the lump remains after redness settles, becomes hard or fixed, or accompanies persistent hoarseness, night sweats, or unexplained weight loss.

Ask whether the lump appears to be in the skin, a lymph node, thyroid, or salivary gland; whether it needs imaging, antibiotics, drainage, or sampling; and which signs mean you should return sooner. If drainage is proposed, ask how to care for the wound afterward.
