Look beyond the loose flakes
Check the skin beneath the scale and the places where it gathers. Loose pale flakes without marked redness may fit dandruff. Seborrheic dermatitis often presents with inflamed areas and oily scale that can extend to the hairline or other oil-rich facial sites. Scale fixed to a sharply edged plaque, circular areas with broken hair, weeping skin or heavy crust can have a different cause. Product build-up may mimic flakes but tends to reflect what was applied and how well it rinsed away.
Care for the scalp without scratching
Use lukewarm water and massage shampoo gently across the scalp with the pads of your fingers. For a medicated wash, respect the package directions and keep it away from the eyes; more product or longer contact is not automatically more effective. Rinse the roots well, clean combs and brushes, and pause fragranced oils, sprays or dyes if they seem to trigger irritation. Avoid lifting adherent scale with nails. If one recommended wash fails or irritates the skin, get advice before combining several active treatments.

Consult a dermatologist when scale persists despite correct product use, spreads beyond the scalp, disturbs sleep through itching or is accompanied by hair breakage. Prompt assessment is appropriate for an increasingly painful scalp, pus, warmth, expanding redness, tender swelling or fever. Obtain urgent assistance after a product reaction that causes breathing trouble, swelling of the lips or tongue, faintness or a fast-spreading blistering rash. Flake size alone does not determine urgency.
Prepare for a scalp assessment
Note where the problem started, whether scale is stuck or loose, and whether there is itch, pain or hair shedding. Photographing a flare before washing can show its distribution. Ask if the pattern fits dandruff, seborrheic dermatitis, contact irritation or another disorder; which active shampoo ingredient to use; and whether topical antifungal treatment belongs in your plan. List dyes, straighteners, oils and scalp medicines used recently.
