What should you know about big black spots on the face?

A dark plug within a pore may be an open comedone associated with acne. A flat mark can remain after inflammation, while a raised, changing, bleeding, or irregularly coloured lesion needs in-person assessment rather than acne treatment.

Open comedones develop when oil and skin cells collect in a pore; the exposed material darkens through contact with air, not trapped dirt. Dark flat patches may follow pimples, folliculitis, shaving irritation, or picking, especially in skin that readily develops pigmentation after inflammation. Deep tender bumps along the jawline may accompany hormonal acne, while bumps centred on hairs can fit folliculitis or razor bumps.

Look for a central pore, elevation, scale, tenderness, itch, and recent change. Also note whether the marks followed breakouts, hair removal, a new cosmetic, or sun exposure. A photograph in natural light can help track evolution.

Wash gently, avoid squeezing or scraping, and choose products labelled non-comedogenic if clogged pores are present. Use broad-spectrum facial sunscreen because ultraviolet exposure can deepen post-inflammatory marks. Introduce only one new active product at a time and stop if burning, swelling, or marked irritation develops.

Do not apply bleach, harsh scrubs, or unlabelled lightening mixtures. A clinician can decide whether treatment should target acne, inflammation, pigmentation, infection, or a distinct skin lesion. Light-based procedures are not interchangeable with routine skin care and should follow a diagnosis, particularly on richly pigmented skin where irritation can worsen uneven colour.

Arrange prompt evaluation for a spot that is new and rapidly changing, develops an uneven border or several colours, bleeds without being picked, repeatedly crusts, becomes persistently painful, or fails to heal. Seek urgent care for facial swelling, spreading redness, fever, severe pain near the eye, or difficulty breathing. These warning signs guide urgency; they do not identify the diagnosis by themselves.

Ask whether the spots are plugs within pores, pigment left after inflammation, or separate lesions. Discuss which ingredients suit your skin, how shaving or cosmetics may contribute, and what changes would require re-examination. If a procedure is proposed, ask how it addresses the diagnosed problem and how irritation and colour change will be minimised.
