Bicornuate uterus with a septum: what the diagnosis means

A bicornuate uterus develops before birth and has a pronounced dip in its outer top surface, creating two upper sections or horns. A septum is tissue that divides part or all of the cavity inside the uterus. The phrase “bicornuate uterus with septum” may describe a complex shape, but it can also reflect uncertainty between two related diagnoses.

Distinguishing them matters because a camera examination inside the uterus shows the cavity but not the outside contour. Three-dimensional ultrasound or pelvic MRI can show both. Some people have no symptoms, while the shape may be discovered during fertility assessment, after pregnancy loss or on pregnancy imaging.

Ask a gynaecologist to review the original images and report rather than relying on the label alone. Clarify the external contour, how far any internal partition extends, and whether the cervix or vagina has an associated variation.

Management depends on symptoms and reproductive plans. Observation may be appropriate when there are no problems. Surgery is not automatic: a uterine septum and a true bicornuate shape are approached differently, so anatomy should be confirmed before discussing a procedure. If you are pregnant or planning pregnancy, arrange obstetric guidance about monitoring the baby's growth, position and signs of early labour.

Seek urgent maternity assessment during pregnancy for vaginal bleeding, leaking fluid, regular painful tightenings, severe abdominal pain, or a clear reduction in your baby's usual movements. Outside pregnancy, sudden severe pelvic pain, fainting, fever or heavy bleeding also needs prompt assessment.

Ask: Does imaging confirm a bicornuate outer contour, an internal septum, or both? Would pelvic MRI or specialist ultrasound clarify the anatomy? Does the finding change pregnancy monitoring? What problem would a proposed procedure address, and what are its alternatives?
