Twin pregnancy with a bicornuate uterus

A bicornuate uterus has two upper cavities. In a twin pregnancy, both babies may be within one cavity or occupy different sides, and ultrasound is used to establish their locations. The team also identifies whether the babies have separate or shared placental structures, because that detail guides the monitoring schedule independently of uterine shape.

Space and uterine shape may influence fetal position, cervical change and the timing of labour. Follow-up therefore focuses on each baby's growth and fluid, the cervix when clinically indicated, placental location and maternal wellbeing. Pelvic MRI may clarify uterine anatomy in selected circumstances, but ultrasound remains central to pregnancy surveillance.

Arrange early obstetric care and attend the additional scans recommended for the twin type and your uterine anatomy. Keep copies of previous imaging, especially reports describing both cavities. Ask your team whom to contact outside clinic hours and which maternity unit is equipped for an earlier-than-planned twin delivery if it becomes necessary.

Delivery planning should consider the presenting baby's position, the second baby's position, placental findings, gestational stage and your health. Discuss vaginal and caesarean pathways before labour, including how the plan could change after an examination or ultrasound.

Contact maternity triage urgently for vaginal bleeding, leaking fluid, regular or intensifying contractions, pelvic pressure with backache, severe abdominal pain, fever, severe headache with visual changes, sudden swelling with feeling unwell, or reduced movement from either baby. Call emergency services for collapse, breathing difficulty, a seizure or uncontrolled bleeding.
