Symptoms of a bicornuate uterus

The upper uterus develops as two joined cavities with a deep external indentation, sometimes described as heart-shaped. Some people report painful periods or pelvic discomfort, but these symptoms have many other possible causes and do not diagnose the uterine shape.

The condition may become relevant after repeated pregnancy loss, early labour, a baby remaining breech, or concerns about fetal growth. These experiences can also occur without a bicornuate uterus. Ultrasound is commonly part of assessment, and pelvic MRI may be used to distinguish this shape from other uterine differences when the anatomy is unclear.

Book a gynaecology review if pelvic pain or menstrual symptoms persist, if you have experienced pregnancy complications, or if a scan suggests an unusual uterine shape. Bring earlier imaging and pregnancy records. The clinician may review the outer contour and inner cavity because this helps separate a bicornuate uterus from a septate uterus, which is managed differently.

If you are planning pregnancy, ask whether the diagnosis is secure and whether any additional assessment is appropriate before conception. During pregnancy, tell the maternity team early so monitoring can be tailored to fetal growth, cervical findings and fetal position.

Seek urgent care for severe or worsening pelvic pain, fainting, heavy vaginal bleeding, fever with pelvic pain, or a positive pregnancy test accompanied by pain or bleeding. In an established pregnancy, contact maternity care urgently for fluid leakage, regular painful contractions before term or reduced fetal movement. These warnings require assessment regardless of uterine shape.

Ask which imaging features confirm a bicornuate uterus, whether the kidneys or urinary tract need evaluation, and whether your current symptoms may have another cause. If pregnancy is planned or underway, ask what monitoring is proposed and what changes should prompt an earlier appointment.
