Bicornuate uterus and fertility

Fertility depends on many factors beyond uterine shape, including ovulation, sperm, the fallopian tubes, age, and other health conditions. A specialist should assess the whole picture rather than assuming the uterine finding explains difficulty conceiving.

A bicornuate uterus has a deep indentation at the top, creating two horn-like spaces. Implantation can occur in either side. The available space and shape may affect how a pregnancy develops or how the baby lies, but individual experiences differ.

Accurate diagnosis is useful because a bicornuate uterus can be confused with a septate uterus, and the management is not the same. Imaging that shows both the cavity and the outside contour helps clinicians identify the anatomy.

If conception is taking longer than expected, arrange a fertility assessment that evaluates both partners when applicable. Take copies of previous ultrasound or MRI reports and describe prior pregnancies, losses, births, pelvic surgery, cycle patterns, and medicines.

Once pregnant, share the diagnosis with the maternity team early. The care plan may include attention to cervical length, fetal growth, the baby's position, and signs of labour, depending on personal history and clinical findings. Surgery to alter uterine shape is reserved for selected situations after specialist evaluation, not offered automatically for infertility.

Seek fertility advice if you are concerned about delays in conception, have irregular or absent periods, have had repeated pregnancy losses, or already know of a reproductive tract difference. The appropriate timing depends on age, health history, and other fertility factors.

In pregnancy, contact maternity services urgently for bleeding, leaking fluid, severe or persistent abdominal pain, regular painful contractions, fainting, or reduced fetal movement once a usual movement pattern is established. These symptoms require assessment regardless of uterine shape.

Ask whether the imaging confirms a bicornuate rather than septate uterus, whether the fallopian tubes and ovaries need separate assessment, and which other fertility factors should be checked. For pregnancy planning, request a clear monitoring schedule and instructions for symptoms that should prompt maternity review.
