Bicornuate and retroverted uterus explained

The bicornuate shape develops before birth when the upper portions of the uterus do not fully join, leaving a deep indentation and two horns. Retroverted describes the direction the uterus points within the pelvis. It is a positional variation and does not make the bicornuate anatomy more divided.

Either feature may be found incidentally during ultrasound. A backward tilt may sometimes be associated with discomfort during sex or pelvic examination, while a bicornuate cavity may be relevant to pregnancy space and fetal position. Pelvic pain can also come from conditions such as endometriosis, fibroids, infection, or ovarian problems, so the scan finding should not automatically be treated as the cause.

If you have no symptoms and are not planning pregnancy, no specific treatment may be required. For persistent pain, a clinician can assess whether the discomfort matches the uterine position or whether another pelvic cause is more likely. Detailed ultrasound or pelvic MRI can confirm the external contour when the diagnosis is uncertain.

Before or during pregnancy, tell the maternity team about the bicornuate shape and provide earlier imaging if available. Follow-up is chosen according to the cavity anatomy, cervical findings, fetal growth and position, and previous pregnancy experience. Retroversion often becomes less noticeable as the enlarging uterus rises out of the pelvis, but individual assessment remains useful.

See a clinician for persistent pelvic pressure, pain during sex, new bladder or bowel difficulty, or menstrual symptoms that disrupt daily life. If pregnant, get urgent help for heavy bleeding, severe pelvic or abdominal pain, fainting, fever, leaking fluid, repeated painful tightenings, inability to pass urine, or reduced fetal movement after a usual pattern has developed. Urgency comes from these symptoms, not from retroversion itself.

Ask how clearly the scan shows the outer uterine contour, whether the cavity shape has been distinguished from a septum, and whether retroversion appears mobile or fixed. If pain is present, discuss other possible pelvic causes. For pregnancy planning, ask whether cervical checks, additional scans, or consultation with a specialist obstetrician would add useful information.
