Bicornuate uterus and didelphys uterus explained

Short answer
A bicornuate uterus has a deeply indented upper cavity, while uterus didelphys involves two separately developed uterine cavities and often two cervixes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Bicornuate uterus
- Two upper uterine horns that join lower down
- Uterus didelphys
- Two more fully separate uterine cavities, often with two cervixes
How the anatomy differs
Both variations develop before birth as the reproductive tract forms. In a bicornuate uterus, the upper uterus has two horns that join into a lower uterine segment. With didelphys, the two sides remain more completely separate; a vaginal partition may also be present. The distinction matters because the shape of the cavity, cervix, and outer uterine contour can influence counselling and pregnancy monitoring.
Either variation may be discovered during imaging, a pelvic examination, investigation of symptoms, or pregnancy care. Some people have no symptoms. Others may report painful periods, difficulty using tampons, pain during sex, or reproductive concerns, particularly when a vaginal partition is present. These symptoms have many possible causes and do not identify the uterine shape by themselves.
How to confirm the diagnosis and plan care
Bring previous scans and pregnancy records to a gynaecology appointment. Three-dimensional ultrasound or pelvic MRI can show both the inner cavity and outer uterine contour, helping distinguish bicornuate anatomy from didelphys and from a uterine septum. A clinician may also check the cervix and vagina if the examination is relevant and acceptable to you.
Treatment is based on symptoms and reproductive history rather than the scan label alone. Surgery is not routinely needed for an incidental finding without problems. If you are pregnant or planning pregnancy, ask whether specialist obstetric follow-up, cervical assessment, or additional monitoring of fetal growth and position is appropriate for your anatomy.
Symptoms that need timely attention
Arrange a gynaecology review for persistent pelvic pain, unusually difficult menstrual flow, pain with penetration, or repeated pregnancy problems. During pregnancy, urgent assessment is appropriate for heavy bleeding, severe or one-sided abdominal pain, fluid leaking from the vagina, regular painful tightenings, fainting, fever, or reduced fetal movement when movements are normally felt. The symptom determines the urgency; the uterine variation alone does not mean an emergency is occurring.
Useful questions at your appointment
Ask which features on imaging establish the diagnosis, how many cervixes are present, and whether there is a vaginal partition. You can also ask whether your kidneys and urinary tract have already been assessed, because reproductive and urinary structures develop at a similar stage before birth, and whether your personal pregnancy history changes the recommended follow-up.

Questions people ask
Are bicornuate uterus and uterus didelphys the same?
No. They arise during similar early development but differ in how fully the two sides join. Imaging of the cavity and outer uterine shape helps separate them.
Can either condition affect pregnancy?
Both can be associated with less space or an unusual cavity shape during pregnancy. The practical effect varies, so care is tailored to the exact anatomy and previous pregnancy history.
