What to know about the bicipitoradial bursa

Short answer
The bicipitoradial bursa cushions the distal biceps tendon near the elbow and can become inflamed, causing deep front-of-elbow or upper-forearm pain.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- Between the distal biceps tendon and the radius near the front of the elbow
- Common aggravator
- Repeated forearm rotation or resisted elbow bending
- Assessment focus
- Pain pattern, swelling, strength and tendon integrity
What bicipitoradial bursitis can feel like
This small fluid-filled sac reduces friction where the distal biceps tendon passes near the radius. Irritation may follow repeated forearm rotation, heavy lifting or a direct impact. Discomfort can worsen when turning a key, using a screwdriver, lifting with the palm upward or bending the elbow against resistance. Some people notice tenderness or fullness at the front of the elbow. Similar pain can arise from the biceps tendon, nearby muscles, a joint problem or a nerve, so location alone does not establish the cause.
Practical steps for relief
Temporarily reduce lifting, pulling and repetitive palm-up or palm-down rotation. Keep the elbow moving gently within a comfortable range rather than holding it completely still. A wrapped cold pack can be used for short periods after an aggravating activity. A clinician can examine resisted movements and forearm rotation, then decide whether imaging is useful. Treatment may include activity changes, physiotherapy and appropriate pain relief; injections or aspiration are considered only after the diagnosis and nearby structures have been assessed.
When to arrange medical care
Seek prompt assessment if the elbow becomes hot, red or rapidly more swollen, especially with fever or feeling unwell. Urgent evaluation is also appropriate after a forceful injury if there was a pop, new bruising, visible change in the biceps shape, marked weakness or inability to use the arm. Book a routine appointment when pain persists, repeatedly returns, interferes with sleep or work, or is accompanied by tingling or numbness.
Useful questions at your appointment
Ask which examination findings separate bursal irritation from distal biceps tendon injury, whether your work or exercise technique is contributing, and which movements you can continue safely. If a procedure is proposed, ask what diagnosis it is intended to confirm or treat, what structures lie nearby and what recovery precautions apply.

Questions people ask
Can bicipitoradial bursitis cause a lump?
An enlarged bursa can create fullness near the front of the elbow, but any new lump should be examined because several structures can produce swelling in this area.
Is imaging always needed?
No. An examination may be enough initially. Ultrasound or other imaging may be considered when the diagnosis is unclear, symptoms persist or a tendon injury or mass needs evaluation.
