Dark nail streaks and the term benign subungual melanoma

A brown or black nail streak may arise from a benign mole, activation of pigment cells, blood after trauma, infection, or medication effects. Subungual melanoma is one diagnosis a clinician considers, but a nail change alone does not identify its cause.

Pigment can originate in the nail matrix, the tissue that produces the nail plate, and then travel outward as the nail grows. A dermatologist looks at which nails are involved, the band’s colour and borders, change over time, nearby skin, medicines, injuries, and the person’s natural pigmentation.

Dermoscopy can reveal details not visible to the unaided eye. When the pattern or history remains concerning, a carefully planned biopsy of the nail unit may be needed. Because the nail matrix is delicate and scarring can alter nail growth, this procedure is best planned by a clinician experienced with nail disorders.

Remove nail polish before the appointment if you can do so without delaying care. Bring older photographs that show the nail, a medication list, and details of any injury or repeated friction. Avoid scraping, bleaching, drilling, or covering the area with artificial nails while the cause is being assessed.

If a clinician considers the finding benign, the plan may include standardized photographs and scheduled review rather than immediate treatment. A bruise should move toward the nail tip as the plate grows; pigment produced in the matrix follows a different pattern. Do not rely on this observation alone when the streak is changing or unexplained.

Seek a timely dermatology appointment for a new or widening dark band, irregular colours or borders, pigment extending onto the surrounding skin, splitting or distortion of the nail, a persistent lump, unexplained bleeding, or a change affecting a single nail. These signs warrant evaluation but are not proof of melanoma.

Urgent care is appropriate for severe throbbing pain after an injury, uncontrolled bleeding, spreading redness, pus, fever, or loss of sensation in the finger or toe. Those features may reflect pressure, infection, or significant trauma and need care for that reason.

Ask where the pigment appears to originate, whether dermoscopic photographs will be used for comparison, and what change should bring you back earlier. If biopsy is proposed, ask which part of the nail unit will be sampled, how discomfort will be managed, and how the procedure might affect future nail growth.
