Can acral lentiginous melanoma be benign?

A pathology report or casual description may contain unfamiliar wording. The exact diagnosis should be confirmed rather than shortened to “benign melanoma,” because that phrase combines conflicting meanings.

Acral refers to the palms, soles and nail units. Acral lentiginous melanoma can present as a new or changing pigmented patch on a palm or sole, or as pigment beneath or around a nail. It may also have little visible pigment.

Benign acral naevi and lentigines can occur in the same regions. A dermatologist assesses the pattern, borders, colours, change over time and dermoscopic features. Appearance alone may not settle an uncertain lesion, so a biopsy can be needed for diagnosis.

Book a dermatology examination for a new, changing or unexplained mark on a palm, sole or nail. Remove nail polish before a nail assessment when practical, and bring older photographs if they show how the area has changed.

Do not cut, burn or treat the mark with bleaching or wart products before assessment. If tissue has already been sampled, request the full pathology wording and ask whether the diagnosis is benign, atypical or malignant and whether the edges were completely assessed.

Arrange prompt review if a mark enlarges, changes colour or shape, bleeds, ulcerates, becomes persistently painful, or fails to heal. Under a nail, concerning changes include a widening dark band, pigment spreading onto nearby skin, nail splitting or distortion without a clear injury.

These signs do not establish melanoma by themselves, but they justify timely examination. A lesion already diagnosed as melanoma should be managed through the treating skin-cancer team rather than observed at home.

Ask for the precise name of the lesion, what examination features support it, whether biopsy is recommended, what type of biopsy preserves diagnostic accuracy, and when results will be discussed. If melanoma is confirmed, ask how its depth and other pathology features guide surgery and further staging.
