Nickel allergy from a belt

The buckle, stud or another metal fitting may be responsible. Symptoms can settle when exposure stops and return after renewed contact. Sweat and friction can make the reaction more troublesome without being the underlying allergy.

Nickel allergy is a delayed immune reaction rather than an immediate poisoning response. The affected skin may become red or darker than the surrounding area, rough, swollen, scaly or blistered. Oozing can occur during an active flare; repeated exposure may leave the patch thickened, cracked and persistently itchy.

A patch centred beneath a buckle is suggestive, but location does not prove the metal content or diagnosis. Irritation from rubbing, trapped sweat, adhesive coatings and laundry products can mimic allergy. Patch testing by a trained clinician can identify nickel sensitivity when the cause remains uncertain or avoidance is difficult.

Remove the belt until the skin recovers. For future wear, choose a buckle specifically verified as nickel-free or use clothing as a complete barrier between skin and every metal part. Replace temporary coatings when damaged, although switching the fitting is more dependable than repeatedly painting it.

Clean the area with lukewarm water and a mild fragrance-free cleanser, then apply a bland moisturiser. Cool compresses can reduce itch. A clinician may recommend a topical corticosteroid of suitable strength for a limited course; choice depends on the site's skin thickness, severity and whether infection is present.

Book an appointment when the diagnosis is unclear, the rash is extensive, it keeps returning despite avoidance, or treatment has not helped. Increasing tenderness, heat, swelling, pus, crusting, fever or red streaking needs prompt assessment for infection. Call emergency services for breathing difficulty, faintness, or swelling of the lips, tongue or throat.
