Belly pain during sex: causes, next steps and warning signs

The location and timing offer useful clues. Pain at the vaginal entrance differs from deep pain felt in the pelvis or lower abdomen, while discomfort that continues after sex may need a different assessment. Note whether it changes with position, your menstrual cycle, urination or bowel movements.

Deep pelvic pain can occur when sensitive tissues are pressed during penetration. Endometriosis can present with deep pain during sex, often alongside painful periods or pain with bowel movements. Pelvic inflammatory disease can present with lower abdominal pain, unusual discharge, bleeding between periods or pain during sex. An ovarian endometrioma may also cause pelvic pressure or pain.

Pain closer to the entrance may relate to dryness, irritation or tightening of the pelvic floor muscles. Genitourinary syndrome of menopause can present with dryness and discomfort during sex. Vulvodynia can present with burning or stinging vulval pain, while lichen sclerosus can present with itching, fragile skin and painful splitting. Symptoms alone cannot establish the cause.

Stop or change the activity if it hurts; continuing through pain can increase muscle guarding and irritation. More time for arousal, a water-based lubricant and positions that let you control depth may reduce pressure. Avoid scented washes or products if the vulval skin feels irritated.

Arrange a routine appointment if the pain returns, disrupts intimacy or comes with menstrual, bladder, bowel or discharge changes. A clinician may ask about the exact pain site, examine the abdomen or pelvis, and consider pregnancy, infection testing or imaging according to your symptoms. Keep brief notes about timing, triggers, bleeding and discharge to make that discussion more precise.

Seek urgent medical help for sudden or severe lower abdominal pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, fever, repeated vomiting or feeling very unwell. Pain with a possible pregnancy needs prompt assessment because pregnancy-related complications must be excluded.

Ask which features point toward a surface, pelvic-floor, uterine or ovarian cause; whether a pregnancy test, infection screen, examination or scan would help; and what pain-relief or lubrication options suit you.
