Not every piercing bump is a keloid. Irritation, infection, an allergic reaction to jewellery, a fluid-filled granuloma, and a hypertrophic scar confined to the wound can look similar, so diagnosis matters before treatment.

Clues that distinguish keloid scar tissue

A keloid can feel smooth, rubbery, or firm and may be skin-coloured, pink, red, brown, or darker than surrounding skin. It often extends past the piercing channel and can itch, ache, or become tender when clothing catches it.

Keloids arise from an exaggerated scar response after skin injury. Personal or family history of keloids and previous raised scars can help the clinician assess likelihood. Pus, increasing heat, spreading redness, or fever instead raises concern for infection.

Protect the piercing and obtain a diagnosis

Avoid cutting, squeezing, tying off, freezing, or injecting the lump yourself. Reduce rubbing from waistbands and do not apply strong acids or unlabelled scar products. If the piercing is still in place, seek clinical advice before removing jewellery because closure can complicate drainage when infection is present.

A dermatologist may discuss steroid injections into the scar, cryotherapy, laser treatment, or surgery, sometimes in combination. Excision alone can be followed by regrowth, so ask how recurrence will be addressed and what aftercare accompanies the chosen procedure.

When the piercing needs prompt review

Arrange an appointment if the lump is enlarging, painful, itchy, repeatedly bleeding, changing colour, or interfering with clothing. Assessment is also useful before another piercing or operation if you have formed raised scars elsewhere.

Seek prompt medical care for rapidly spreading redness, increasing warmth or swelling, pus, red streaks, fever, or feeling generally unwell. Sudden severe pain or darkening skin around tight or embedded jewellery also warrants urgent examination.
