Induction at 38 weeks: what to expect

Short answer
At a 38-week induction, expect an initial assessment followed by cervical preparation or labour-starting treatment tailored to your examination.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First assessment
- The team reviews maternal observations, the baby and readiness of the cervix.
- Possible methods
- Medicine, a balloon, breaking the waters and oxytocin may be used in sequence.
- Timing
- Waiting and reassessment can be part of the process before active labour begins.
The likely sequence
Admission commonly includes a review of the reason for induction, checks of your observations, assessment of the baby's position and heartbeat, and discussion of consent. The process may contain pauses while a treatment works or while the team reassesses the cervix. Your exact pathway depends on findings and how you and the baby respond.
From cervical preparation to contractions
When the cervix is closed or firm, medicine or a balloon catheter may be used to soften and open it. If the cervix is ready, a clinician may offer to break the waters. Oxytocin through a drip may follow if contractions do not begin or need strengthening. Monitoring helps the team judge the baby's heartbeat, contraction frequency and whether treatment should be adjusted.
Induced contractions can feel intense, but pain-relief choices remain available. Movement, breathing techniques, water, injected medicines or an epidural may be options depending on the stage of labour, your preferences and the unit's facilities.
How to take part in your care
Tell staff about allergies, previous birth experiences, current medicines and what matters to you during labour. Ask before eating or drinking because advice may change with the clinical plan. Use the call bell for a sudden change in pain, bleeding, fluid colour, pressure or the baby's movements. Rest and eat when permitted during quieter periods, since the timetable can shift.
If progress is slower than expected, ask what the examination shows and what choices come next. Those choices can include more time, another method, reassessment of the original reason for birth, or caesarean delivery when vaginal birth is no longer considered the safer course.
Changes to report immediately
Before admission, call maternity triage for reduced or altered fetal movement, vaginal bleeding, broken waters or labour starting. During induction, alert staff immediately to heavy bleeding, constant severe abdominal pain between contractions, breathing difficulty, chest pain, faintness, a severe headache, visual disturbance or a strong urge to push. Staff can assess whether the planned sequence needs to change.
Questions before treatment starts
Ask which cervical preparation is suitable, when examinations are likely, what monitoring permits movement, and which pain relief can be requested at each stage. Find out who will update you during delays, what findings would lead to an oxytocin drip, and how the team defines unsuccessful induction in your circumstances.

Questions people ask
Will I be in active labour as soon as induction starts?
Not necessarily. Cervical preparation works before active labour for some people, and the team may reassess before offering the next method.
What happens if the first induction method does not work?
The maternity team reassesses you, the baby and the cervix, then discusses options such as waiting, another method or caesarean birth according to the clinical situation.
