What to do after chickenpox exposure in pregnancy

Do not wait for a rash. A clinician can clarify whether the contact was significant, check your history or arrange an immunity blood test, and decide whether preventive treatment is appropriate. Call before attending a clinic so staff can prevent exposing other pregnant people or newborn babies.

Chickenpox is caused by varicella-zoster virus and spreads readily from a person with active infection. Previous chickenpox or vaccination may provide immunity, but memory alone is not always enough to guide care; a blood test can clarify protection when the history is uncertain.

Shingles is the same virus becoming active again in someone who previously had chickenpox. Contact with fluid from uncovered shingles blisters can cause chickenpox in a person who is not immune, rather than causing shingles. Tell your clinician whether the source had chickenpox or shingles and whether the rash was covered.

Be ready to explain when and where the contact occurred, how close and prolonged it was, and when the other person’s rash began. Share whether you have had chickenpox, a chickenpox vaccine, an earlier immunity result, or medicines or conditions that weaken immune responses.

If testing shows that you are not immune, your clinician may offer time-sensitive preventive treatment. If illness develops, antiviral treatment may be considered, so report a new rash without delay. Chickenpox vaccine is not given during pregnancy; if you remain non-immune, discuss vaccination after pregnancy. Avoid further contact with chickenpox and with exposed pregnant people or newborns while following your care team’s instructions.

Contact your maternity team or doctor urgently if you develop fever, feel unwell, or notice an itchy rash that becomes fluid-filled blisters. State that you are pregnant and may have chickenpox before going to any healthcare setting.

Seek emergency care for breathing difficulty, chest pain, severe headache, unusual drowsiness, repeated vomiting, vaginal bleeding, a severe or bleeding rash, or rapidly worsening illness. These can indicate complications requiring hospital assessment. Tell emergency staff about the possible infection before arrival whenever practical.

Ask whether your contact meets the threshold for exposure, whether an immunity blood test is needed, and when you should expect the result. If you are not immune, ask which preventive option suits your stage of pregnancy and health history, when it must begin, and what follow-up you and the baby may need if a rash appears.
