Beginning stages of chickenpox: what to expect

The rash often starts as small red spots, then forms fluid-filled blisters that crust over. New spots can appear while older ones are blistering or crusting, so several stages may be visible together.

Chickenpox is caused by the varicella-zoster virus and spreads readily to people who are not immune. Spots often appear first on the chest, back, scalp, or face and may extend elsewhere, including inside the mouth.

Other infections and skin reactions can also blister. A clinician may need to assess an unusual rash, especially after medicine exposure or when spots are painful rather than mainly itchy.

Stay away from school, work, and public places until every spot has crusted, and avoid contact with pregnant people, newborns, and anyone with a weakened immune system. Call ahead before visiting a clinic so staff can prevent exposure in the waiting area.

Offer fluids, keep nails short, use loose clothing, and discourage scratching. Ask a pharmacist or clinician which fever or itch medicines are suitable for the person's age and medical history; some common medicines should not be used in chickenpox without professional advice.

Contact a clinician promptly if chickenpox is suspected during pregnancy, in a newborn, in an adult, or in someone with reduced immunity. Early advice matters because preventive or antiviral treatment may be considered for some people.

Seek urgent care for breathing difficulty, confusion, unusual sleepiness, a stiff neck, severe headache, repeated vomiting, trouble walking, dehydration, or skin that becomes very red, hot, swollen, or increasingly painful around the spots.

Ask whether the rash fits chickenpox, whether testing is needed, which symptom medicines are safe, and whether an antiviral is appropriate. Mention pregnancy, immune conditions, long-term medicines, vaccination history, and recent contact with a vulnerable person.
