Beginning signs of Stevens–Johnson syndrome

Short answer
Stevens–Johnson syndrome may begin with fever and flu-like illness, followed by painful skin, a spreading rash, blisters, or sores affecting the mouth, eyes, or genitals.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Recognise the warning pattern
Red, purple, dusky, or target-like patches may appear before the skin blisters or peels. Eye pain, gritty eyes, light sensitivity, lip crusting, painful swallowing, or painful urination can reflect mucosal involvement. Skin pain and a rapidly changing rash are especially concerning.
Why the pattern is an emergency
Stevens–Johnson syndrome is a severe reaction that damages the skin and moist linings of the body. Medicines are a frequent trigger, although infections or an unclear trigger may be involved. Symptoms can begin while taking a medicine or after a recent course.
Many ordinary drug eruptions are milder, but a painful rash with blistering, peeling, facial swelling, or mouth, eye, or genital sores requires emergency assessment. The urgency comes from these red flags, not from trying to judge the likelihood at home.
What to do immediately
Go to an emergency department now or call local emergency services if this warning pattern is present. Do not drive yourself if you feel faint, confused, short of breath, or severely unwell. Bring all prescription medicines, over-the-counter products, supplements, and their packaging, plus the dates each was started or stopped.
Do not restart a suspected medicine or take another dose merely to test the reaction. A clinician must decide which medicines should be stopped or continued, because abruptly stopping some essential treatments also carries risk. Avoid applying new creams or breaking blisters while arranging care.
When emergency assessment is needed
Treat a rapidly spreading or painful rash, blistered or peeling skin, mouth ulcers, swollen lips, eye pain, vision changes, breathing difficulty, or genital sores as an emergency, particularly after a new or recently changed medicine. Waiting for a routine clinic appointment is not appropriate for these symptoms.
Hospital teams may involve dermatology, eye care, and other specialists because the reaction can affect several body surfaces. Treatment focuses on removing the trigger when safe, supporting fluids and pain control, protecting affected tissue, and managing complications.
Questions after urgent treatment begins
Ask which medicine or infection is suspected, which related drugs must be avoided, how the reaction should be recorded, and whether you need an allergy alert. Confirm the follow-up plan for eyes, mouth, skin, and genital symptoms, and ask what changes should trigger an immediate return.

Questions people ask
Can Stevens–Johnson syndrome begin like flu?
Yes. Fever, fatigue, sore throat, cough, or burning eyes can precede the painful rash and mucosal sores, but these early symptoms are not specific on their own.
Is every rash after a new medicine Stevens–Johnson syndrome?
No. Drug rashes have many causes and presentations. Pain, blisters, peeling, or involvement of the mouth, eyes, or genitals are emergency warning signs regardless of the final diagnosis.
Should I wait to see whether blistering settles?
No. Blistering, peeling, painful skin, or mucosal sores need immediate emergency assessment because the condition can progress and affect several organs.
