Why is there no visible head?

The absence of a head does not show whether the lump is infected. Increasing tenderness, heat, redness, swelling and feeling unwell are more useful warning features than the appearance of a surface point.

How a deeper swelling forms

Each Bartholin gland drains through a narrow duct beside the vaginal opening. When that duct becomes blocked, fluid collects under the surface and creates a lump on one side. A small cyst may cause little discomfort, while a larger one can create pressure during walking, sitting or sex.

An infected collection is called an abscess. It can become intensely sore and tense without developing a pimple-like tip. Not every lump in this area comes from a Bartholin gland; skin cysts, inflamed follicles and other vulval conditions can look similar.

What you can do now

Use comfortably warm water soaks to soothe the area and encourage natural drainage. Choose loose clothing, reduce activities that rub the swelling and keep the skin clean with gentle rinsing. Do not squeeze, puncture or repeatedly check the lump by pressing it.

Have it examined if discomfort persists, the swelling gets larger or the same problem returns. Treatment depends on the size, symptoms and signs of infection. When drainage is needed, a clinician can create a controlled opening and may place a small catheter to keep it from sealing too soon.

Changes that need prompt attention

Get urgent medical help if pain escalates quickly, redness spreads, swelling grows rapidly, you develop fever or you feel generally unwell. Being unable to sit, walk or pass urine comfortably because of the swelling also warrants prompt assessment.

A persistent new lump should be checked even if it causes no discomfort, especially after menopause. An examination can confirm its location and determine whether imaging, a sample or treatment is appropriate.
