What to do if a Bartholin cyst bursts on its own

Fluid release often reduces pressure and pain. The opening may continue to ooze while the cavity empties, and the swelling may gradually become smaller. Spontaneous drainage does not show whether the original lump was a simple cyst or an abscess, and the duct can block again after the opening closes.

Improving tenderness and decreasing swelling are reassuring changes. Thick pus, a strong unpleasant smell, continued or increasing pain, surrounding heat or redness, fever, or feeling unwell can indicate ongoing infection. Fresh bleeding that is heavy or does not settle also needs assessment.

A residual lump may mean that the cavity has not emptied fully or that another type of vulval swelling is present. Examination is useful if the area fails to improve or the diagnosis was never confirmed.

Rinse gently with warm water or sit in a shallow warm bath, then pat the skin dry. A clean, unscented pad or liner can absorb fluid; replace it whenever damp. Wash your hands before and after touching the surrounding skin and wear loose underwear to reduce friction.

Do not squeeze the lump to force out more fluid, insert anything into the opening, or apply antiseptics, perfumes or unprescribed creams. Avoid penetrative sex while the area is open, sore or draining. If discomfort requires medicine, check the label and confirm suitability with a pharmacist when you have other conditions, take other medicines or could be pregnant.

Seek urgent help for severe or worsening pain, fever, rapidly spreading redness, increasing swelling, faintness, difficulty passing urine, or feeling significantly unwell. Drainage may be incomplete, and an abscess can require a procedure or treatment for infection.

Arrange a routine appointment if oozing continues without improvement, the opening closes while a painful lump remains, or the swelling comes back. A clinician may discuss a catheter or marsupialisation to maintain drainage for a recurrent cyst. A new lump after age 40 or after menopause merits prompt examination even if it has already drained.

Ask whether the cavity has drained adequately, whether the fluid or tissue needs testing, and how long to use a pad while it heals. Clarify which activities can resume, what signs suggest the opening has sealed too early, and whether repeated episodes make a procedure worth considering.
