Seek prompt assessment for progressive difficulty swallowing, painful swallowing, repeated vomiting, unexplained weight loss, black stools, vomiting blood, or worsening chest or upper abdominal pain. Escalation is based on these warning features, not on whether reflux is common.

Get emergency help if food is completely stuck and you cannot swallow saliva, if bleeding is heavy, or if you feel faint, severely weak, or short of breath. Sudden severe chest pain also needs urgent assessment.
