Barking cough with high fever: what to know

Fever and cough can occur together with viral illnesses such as a common cold, acute bronchitis, or bronchiolitis. A harsh cough may reflect swelling around the upper airway, while fever suggests the body is responding to infection. The combination does not identify one diagnosis. Age, vaccination history, other medical conditions, the fever pattern, and whether breathing is noisy at rest all influence the assessment. A child who is drinking and alert may be managed differently from someone who is exhausted or dehydrated.

Measure temperature using a reliable method and write down when fever medicine was given, if used according to the label or a clinician’s advice. Encourage frequent sips of fluid and allow rest. Keep the person away from smoke and avoid forcing food. Do not use leftover antibiotics. Arrange medical advice promptly if fever is high or persistent, the cough is worsening, or the person has asthma or another breathing condition.

Call emergency services for severe breathing difficulty, blue or grey lips, pauses in breathing, inability to swallow, drooling, confusion, collapse, or a child whose chest pulls in with breaths. Seek same-day assessment when breathing is faster than usual, fluids are not staying down, urine becomes much less frequent, or the person is unusually drowsy. Fever with a barking cough in a very young child deserves a low threshold for clinical advice.
