What to know about baricitinib and FDA approval

FDA approval applies to a defined use and formulation; it does not mean the medicine suits every type of hair loss or every patient.

Baricitinib is an oral Janus kinase inhibitor that changes immune signalling. In alopecia areata, the immune system attacks hair follicles, and reducing selected signals can allow regrowth in some adults with severe disease.

The US indication is specific to severe alopecia areata in adults. A clinician still needs to confirm the diagnosis and judge whether expected benefit outweighs individual risks. Approval by the US regulator is also separate from medicine registration and prescribing rules in the UAE.

Tell the prescriber about previous blood clots, heart or blood-vessel disease, cancer, recurrent infections, tuberculosis exposure, liver or kidney problems, pregnancy plans and every medicine or supplement you take. Vaccination needs should be reviewed before treatment.

Baseline checks commonly include blood counts and assessment for tuberculosis, hepatitis, liver function and kidney function. Keep scheduled laboratory monitoring, do not change the dose yourself and avoid combining baricitinib with another potent immune-suppressing treatment unless the specialist has deliberately planned it.

Contact the treating team promptly for fever, a persistent cough, shingles-like blisters, unusual bruising or a new skin change. These can signal infection, blood-cell changes or another problem needing assessment.

Seek urgent medical help for chest pain, sudden breathlessness, coughing blood, one-sided leg swelling, sudden weakness, facial droop or difficulty speaking. Do not take another dose while waiting for advice if a clinician suspects a serious infection.

Ask how severe alopecia areata was confirmed, which baseline checks you need, how response will be judged, which symptoms require stopping treatment and whether your vaccines or other medicines need adjustment.
