What is happening in the foot

Bursae are small fluid-filled cushions that reduce friction where tissues move over prominent areas. When a bursa beneath or around the forefoot becomes irritated, pushing off the ground may produce a focused, aching or burning pain. The spot can feel tender when pressed, and footwear may become uncomfortable.

Similar discomfort can come from a stress injury, nerve irritation, an inflamed joint or changes in the fat pad. A clinician may compare both feet, locate the tender structure and assess gait, footwear and joint movement before deciding whether imaging is useful.

Reducing irritation safely

Temporarily reduce running, jumping and long periods of standing while keeping within comfortable activity. Choose shoes with a roomy toe box and supportive, cushioned sole. A correctly positioned metatarsal pad may shift load away from the sore point; placing it directly beneath the painful area can increase pressure.

A wrapped cold pack can be used briefly after activity if it feels soothing. Medication is not suitable for everyone, especially people with stomach, kidney, bleeding or medication-interaction concerns. If symptoms continue, management may include footwear modification, guided rehabilitation or treatment aimed at the confirmed cause.

When the foot needs assessment

Arrange an assessment if pain keeps returning, limits normal walking, follows a change in training, or does not settle with pressure reduction. Seek prompt care after significant injury, or when you cannot bear weight. Redness, spreading warmth, drainage, fever or rapidly increasing swelling require urgent evaluation because infection or another acute problem may be present.

People with diabetes, reduced sensation, poor circulation or an open skin wound should have new forefoot swelling or skin changes checked early.
