Red Spots on a Bald Head: Possible Causes and Next Steps

Look for clues such as whether each spot centres on a hair follicle, contains pus, follows shaving, feels itchy or painful, or forms a broader scaly patch. These details guide the likely explanation but do not replace an examination.

Folliculitis presents with inflamed bumps around follicles and can be tender or pus-filled. Razor bumps occur when cut hairs grow back into the skin, especially after a close shave. Acne can produce blocked pores, whiteheads or deeper sore bumps. Contact irritation tends to appear where a new cleanser, sunscreen, dye or shaving product touched the scalp.

On brown or black skin, inflammation may look deep red, violet or darker than nearby skin rather than bright red. Marks can remain after the active bump settles.

Pause close shaving over active bumps and cleanse with lukewarm water and a mild fragrance-free product. Do not squeeze, scratch or repeatedly scrub the area. Use clean pillowcases, hats and grooming tools, and avoid sharing clippers.

If a recently introduced product caused burning or a new eruption, stop it and note the ingredient list. Photographing changes in consistent light can help a clinician judge whether the area is spreading or healing. Treatment depends on whether the cause is irritation, ingrown hair, acne, infection or another inflammatory condition.

Seek prompt medical care for rapidly expanding redness, marked warmth, swelling, severe pain, extensive pus, red streaking, fever, or swelling around the face or eyes. Arrange a routine review when spots recur, persist, form scars, bleed without an obvious shaving nick, or accompany unexplained hair loss.

Ask whether the spots are follicle-based, whether infection testing is needed, which products to pause, and how to restart shaving safely. Tell the clinician about itch, pain, drainage, hair loss, recent sun exposure and any new scalp products or medicines.
