What should you know about bald head flaky scalp?

Without hair, loose scale and redness are easier to see. Fine dry flakes after harsh cleansing suggest a different problem from greasy widespread scale, thick defined plaques, or a localised scaly area with broken hairs at the edge of a partially bald patch.

Dandruff tends to produce loose flakes with limited inflammation. Seborrheic dermatitis can cause greasier scale and redness and may also affect the eyebrows, beard area, or folds beside the nose. Scalp psoriasis often presents with thicker, more sharply outlined scale and may accompany plaques elsewhere or nail changes.

Scalp ringworm is an infection that can present with scale and patchy hair loss; inflamed, tender, boggy areas need prompt evaluation. Product reactions and razor irritation may burn or itch soon after exposure. Because several patterns overlap, persistence or focal change is more useful than guessing from flake colour alone.

Switch to lukewarm water and a fragrance-free gentle cleanser, then pat the scalp dry. Stop new oils, dyes, aftershaves, or close shaving if they coincide with symptoms. Do not scrape scale, share razors, or use concentrated essential oils. Wash hats and pillowcases regularly if they collect sweat or product residue.

A medicated shampoo may help dandruff or seborrheic dermatitis when used on the skin for the labelled contact time. It will not treat every cause of flaking. Suspected scalp ringworm generally requires clinician-directed treatment rather than relying on shampoo alone, and thick psoriasis may need a tailored prescription regimen.

Arrange prompt care for a painful swollen patch, pus, spreading redness, fever, tender neck glands, or rapidly worsening inflammation. These findings can accompany infection and should not be managed by repeatedly changing cosmetic products.

Book a routine dermatology review when flaking does not settle with gentle care, returns quickly, forms thick plaques, or comes with unexplained patchy hair loss. A persistent rough, crusted, bleeding, or non-healing area on a sun-exposed bald scalp also needs examination.

Ask whether the scale pattern suggests dandruff, seborrheic dermatitis, psoriasis, ringworm, or contact irritation. Clarify whether a skin scraping or another test is needed, how each prescribed product should be applied to exposed scalp, what can be used between treatment days, and when lack of improvement should trigger reassessment.
