A Bag Under One Eye: Causes and Next Steps

One-sided under-eye fullness can come from temporary fluid retention after sleeping on that side, rubbing, allergy, a bite, or irritation from a product. A blocked eyelid gland or local infection may cause a more defined tender swelling. Natural differences in facial structure can also make a tear-trough hollow or lower-eyelid fat pad cast a stronger shadow on one side.

Long-standing loose eyelid skin, known as dermatochalasis, may contribute to asymmetry. The useful clues are when the change began, whether it fluctuates, and whether there is itching, tenderness, a lump, skin colour change, discharge, or altered vision.

For mild puffiness without pain or eye symptoms, use a cool clean compress, avoid rubbing, and stop any new cosmetic that irritates the area. Keep the head slightly raised during sleep and note whether the swelling settles or returns. Do not squeeze a lump or place ice directly on thin eyelid skin.

Persistent cosmetic asymmetry needs diagnosis before treatment. Filler can soften selected hollows but may worsen puffiness in an unsuitable candidate. Eyelid surgery addresses different structural concerns and requires specialist assessment. Bring older photographs to a consultation so the clinician can compare the current appearance with your baseline.

Seek urgent medical care if swelling is accompanied by reduced or double vision, severe eye pain, pain when moving the eye, marked redness, fever, a bulging eye, a severe headache, facial weakness, or swelling after significant injury. Rapid progression or trouble breathing also needs emergency help.

Book a routine assessment if the bag persists, repeatedly affects the same side, develops a firm lump, or comes with ongoing watering, discharge, or skin inflammation. A clinician may examine the eyelid, eye surface, sinuses, and surrounding skin rather than treating the visible contour alone.

Ask whether the fullness is mainly fluid, inflammation, a gland problem, skin laxity, a hollow, or protruding fat. If considering a cosmetic procedure, ask how it could affect asymmetry, what eye examination is needed, which side effects warrant urgent contact, and whether observation is a reasonable choice.
