What should you know about bad headache and nosebleed?

A nosebleed often starts when small blood vessels inside the nose are irritated by dry air, rubbing, nose picking, allergies, or a cold. Congestion and facial pressure from sinusitis may occur at the same time as a headache. Hypertension does not explain every nosebleed, but very high blood pressure can be associated with headache or other symptoms and needs medical review. The two symptoms may also happen together by coincidence, so their timing and severity are useful details.

Sit upright, lean slightly forward, and breathe through your mouth. Pinch the soft part of the nose continuously while keeping the head forward; do not tilt back or lie flat. Spit out blood rather than swallowing it. Avoid blowing or picking the nose, strenuous activity, hot drinks, and alcohol for a while after bleeding stops. Note how long the bleeding lasts, whether it follows an injury, and any medicines that affect bleeding.

Get emergency help for a sudden severe headache, weakness, confusion, fainting, seizure, chest pain, trouble breathing, or a nosebleed that is heavy or will not stop with firm pressure. Seek urgent assessment after a significant head injury, with repeated bleeding, or when headache and nosebleed occur with vision change or marked illness. Arrange a review for frequent episodes, easy bruising, or symptoms that keep returning.

Ask whether the nose needs examination, whether blood pressure should be checked, and how to prevent repeat bleeding. Tell the clinician which nostril bled, the duration and amount, headache onset and location, recent infections or allergies, injuries, and all medicines or nasal products used.
