Managing severe dandruff on the scalp

The amount of visible flaking does not by itself show how serious a scalp problem is. Pain, broken skin, discharge, swelling, and changes in hair growth provide more useful reasons to arrange care.

Dandruff usually produces loose white or grey flakes without marked scalp damage. Seborrheic dermatitis can present with greasier scale and inflamed skin, including around the ears, eyebrows, or sides of the nose. Thick, well-defined plaques or scale extending beyond the hairline may fit scalp psoriasis and need a different plan.

Residue from styling products can mimic flakes, while hair dye or fragranced products may trigger irritation. Picking can then add crusts that obscure the original pattern.

Wash often enough to remove oil and loose scale without aggressively scrubbing. Work a suitable medicated shampoo onto the scalp, leave it for the label's stated contact time, and rinse completely. Conditioner can be placed on the hair lengths if dryness is a concern.

Introduce one treatment change at a time so irritation can be traced. Stop a product that causes burning, marked redness, or swelling. Avoid fingernails, stiff combs, and concentrated essential oils on inflamed skin.

Arrange an appointment when consistent shampoo use does not control the problem, when the diagnosis is uncertain, or when scale is thick and attached. Earlier review is appropriate for bald patches, broken hairs, tender pustules, or recurrent sores. Rapidly spreading redness, fever, drainage, severe pain, or facial swelling requires prompt medical attention.

Ask which diagnosis best matches the scale and whether more than one scalp condition is present. Request clear instructions about shampoo contact time, frequency, maintenance use, and whether a topical antifungal or anti-inflammatory treatment belongs in the plan.
