Why it may happen
Calf cramps can follow exercise, prolonged standing, an awkward position, or dehydration. A muscle strain may cause soreness that lasts beyond the cramp. Repeated cramps can also occur alongside circulation problems, thyroid disease, reduced muscle mass, vitamin D deficiency, or kidney disease. A cramp alone does not establish any of these conditions.
Pay attention to whether one calf is swollen, warm, red, weak, or painful between episodes. Also mention new medicines, heavy sweating, vomiting, diarrhoea, or a recent change in activity.
What to do during a cramp
Stop the activity and gently straighten the knee while drawing the toes toward the face, or stand and place weight gradually through the affected leg if safe. Massage the muscle lightly and use comfortable warmth afterwards. Replace fluids after sweating, but do not take salt or mineral supplements routinely without advice. Regular calf stretching and supportive footwear may reduce triggers for some people.

Seek urgent help for a swollen, hot, red calf; sudden unexplained one-sided pain; chest pain; or breathlessness. Arrange a review if cramps are frequent, wake you repeatedly, cause weakness, follow a medicine change, or do not settle.
