Bad cough with no fever: what should you know?

Fever is not required for a cough to be troublesome. A recent cold or acute bronchitis may leave the airways sensitive after other symptoms settle. Post-nasal drainage from allergic rhinitis can trigger throat clearing and coughing, while acid reflux can cause a dry cough, especially after meals or when lying down. Asthma may present mainly as cough or chest tightness. Bronchiectasis can produce a long-lasting cough with mucus. In children, bronchiolitis has different warning signs and needs age-sensitive assessment.

The pattern matters: note whether the cough is dry or brings up mucus, whether it follows exercise or meals, and whether it disturbs sleep.

Drink fluids regularly, rest your voice, and avoid smoke, vaping, dust, and strong fragrances. Warm drinks may soothe an irritated throat. Keep the room comfortably humidified without exposing anyone to burns from hot steam. Follow label directions for any over-the-counter product and check with a pharmacist if the person is pregnant, a child, or takes regular medicines. Arrange a clinical review if the cough is worsening, persists, or affects breathing or sleep.

Seek urgent help for severe breathlessness, blue or grey lips, confusion, fainting, chest pain, or inability to speak in full sentences. Prompt assessment is also sensible when wheezing is new, the cough follows choking, or a child is struggling to breathe. Blood in the mucus, dehydration, or a cough that keeps returning warrants medical review even without fever.
