Bacterial vaginosis odour: what it means and what helps

Bacterial vaginosis, or BV, develops when the usual balance of vaginal bacteria changes. It often presents with a stronger fish-like smell and a thin grey-white discharge; the odour may be more noticeable after sex or around menstruation. Marked soreness and intense itching are less characteristic and may point toward another or additional condition. BV is not classified as a sexually transmitted infection, although sexual activity can influence the vaginal bacterial balance. Odour alone cannot rule out trichomoniasis or another infection.

Arrange an assessment when the smell or discharge is new, persistent, recurrent, linked with a new sexual partner, or accompanied by irritation. A clinician may examine the discharge and take a vaginal swab to distinguish BV from yeast, trichomoniasis and other infections. Prescribed antibiotic treatment may be taken by mouth or used vaginally, depending on the medicine and your circumstances. Complete treatment as directed. Avoid douching, fragranced washes and deodorising products because they can irritate tissue and disturb the vaginal environment further. Wash the outer vulva gently and keep products out of the vagina.

Get prompt medical advice if odour or discharge occurs during pregnancy, after a gynaecological procedure, or with fever, pelvic pain, pain during sex, pain when passing urine, sores or unexpected bleeding. Seek urgent help for severe pelvic or abdominal pain, fainting, heavy bleeding, repeated vomiting or feeling acutely unwell. Return to the clinician if symptoms remain after the prescribed course or quickly recur, because the diagnosis or treatment plan may need reconsideration.

Ask whether your discharge should be tested, which treatment route suits you and what medicine interactions or side effects to watch for. Mention pregnancy possibility, breastfeeding, medication allergies, recent antibiotics and recurrent episodes. If symptoms keep returning, discuss whether another infection has been excluded and whether any current vaginal products or practices could be contributing to irritation or bacterial imbalance.
