When a back spasm will not go away

Why tightening can continue

Keep a brief symptom record covering onset, recurrence, sleep interruption, leg symptoms, medicines tried, and effects on work or self-care. Bring it to a doctor or physiotherapist. The assessment may examine spinal and hip movement, muscle control, reflexes, sensation, and strength before a treatment plan is selected.
Choose urgent or routine review
Obtain urgent medical help if persistent spasm is joined by new bladder or bowel control changes, numbness around the saddle area, worsening leg weakness, unsafe walking, fever with severe pain, or symptoms after major trauma. These features require assessment rather than further home treatment.
Schedule a routine appointment when improvement has stalled, episodes are becoming more frequent, sleep is repeatedly disrupted, or normal activities remain restricted. Tell the clinician if pain is constant at rest, extends down a leg, or is accompanied by unexplained illness.
