The short answer on stretching

During the strongest phase, easy walking, breathing, and small position changes may be better tolerated than a long static stretch. The appropriate movement should leave the back the same or calmer afterward.

Why a stretch may help or irritate

A spasming muscle is contracting involuntarily and may be protecting a sensitive area. Slow movement can reduce fear and stiffness, while a forceful end-range stretch can make the nervous system increase guarding. More intensity is therefore not a useful target.

The direction that feels restricted is not automatically the direction to push. A muscle strain can present with local pain during loading, while a herniated disc can present with leg symptoms that change during spinal movement. The response across the whole symptom area should guide what you do.

Use a controlled approach

Begin in a stable position and breathe normally. Try a small pelvic tilt while lying with knees bent, or gently move the knees a short distance from side to side. For the upper back, an easy supported reach may feel better than pulling firmly on the arm.

Move slowly, stay within a comfortable range, and return to the starting position without bouncing. Stop if pain becomes sharp, travels farther into an arm or leg, or produces tingling, numbness, weakness, dizziness, or stronger spasm. Do not have another person force the movement.

Combine mobility with short walks and normal daily activity as tolerated. When the acute guarding settles, strengthening and graded exposure to bending or lifting are usually more relevant to restoring capacity than repeatedly chasing flexibility.

When to stop and seek care

Do not continue stretching after significant trauma or when the spasm is accompanied by fever, severe unwellness, new bladder or bowel control difficulty, saddle-area numbness, or marked limb weakness. These symptoms need urgent medical assessment.

Seek a routine clinical review when each attempt at movement makes symptoms spread, pain is worsening rather than settling, sleep is repeatedly disrupted, or spasms keep returning. A tailored examination can identify safer directions and an appropriate loading plan.

Ask which movements centralise or reduce your symptoms, whether any direction should be limited temporarily, and when to add resistance. Report what happens during the movement and later, rather than describing only how far you can stretch.
