Call emergency services for sudden upper-back pain with chest pressure, shortness of breath, sweating, faintness, confusion, coughing blood, or pain spreading to the jaw or arm. Sudden severe tearing pain, particularly with collapse or new weakness, also requires emergency help.

Obtain prompt assessment after a fall or direct blow, or when pain occurs with fever, a new rash, painful breathing, progressive arm weakness, poor coordination, or unexplained illness. Persistent pain that disrupts sleep or does not respond to sensible activity changes deserves a clinical review.
