Possible explanations

Dehydration, missing meals, a drop in blood pressure on standing, an inner-ear problem, illness, anxiety, or a medicine side effect can cause dizziness while a muscle or joint problem causes the back pain. Pain itself can also make some people feel faint or unsteady.

Less often, the combination appears with a problem affecting the brain, spinal nerves, circulation, or internal organs. The timing gives useful clues: note whether dizziness begins after standing, follows a new medicine, accompanies fever, or starts at the same moment as sudden severe pain.

Stay seated or lying down until the sensation settles, then rise in stages while holding a stable surface. Drink water if you are normally allowed fluids. Do not climb, use machinery, exercise alone, or stretch forcefully while balance is impaired.

Arrange a clinical appointment if dizziness persists, repeatedly returns, began after a medicine change, or limits walking and daily activity. Record the sensation, duration, position at onset, back-pain location, and associated headache, hearing change, nausea, palpitations, numbness, or weakness.

Call local emergency services for fainting, new facial droop, trouble speaking, confusion, a sudden severe headache, new weakness or numbness on one side, chest pain, breathing difficulty, or sudden severe back pain with a cold or clammy feeling.

Go for emergency assessment if back pain occurs with new inability to walk, loss of bladder or bowel control, numbness around the groin, or rapidly worsening weakness in the legs. Fever with severe back pain, especially after a recent infection or procedure, also needs prompt medical evaluation.
