Swelling behind the knee: what to know

A soft or tight fullness behind the knee can come from fluid related to irritation inside the joint, a muscle or tendon problem, or a collection of fluid often called a Baker cyst. Visible surface veins and aching after standing may fit Varicose Veins. Swelling in both legs can be associated with fluid balance problems, including Heart Failure or Chronic Kidney Disease, although those conditions have other features too. Obesity can increase pressure on joints and veins.

The location does not identify the cause. Note whether the swelling is one-sided, warm, red, tender, or changing, and whether the knee is difficult to bend.

Avoid activities that increase pain, but keep gently mobile if movement is comfortable. Elevate the leg when resting, avoid tight clothing around the knee, and use a cool pack wrapped in cloth for a short period after a minor strain. Do not massage a painful, warm, or suddenly swollen leg until a clinician has assessed it. Arrange an appointment if the swelling persists, returns, follows an injury, or affects walking. A clinician may examine the knee and circulation and decide whether imaging is needed.

Get emergency help if leg swelling is accompanied by sudden breathlessness, chest pain, coughing blood, faintness, or a racing heartbeat. Seek same-day assessment for sudden one-sided swelling, warmth, redness, marked tenderness, or rapidly increasing pain, particularly after prolonged immobility, travel, surgery, or an injury. Seek prompt care for a cold or pale foot, new weakness, or inability to bear weight.

Ask what structure appears swollen, whether a blood clot or joint fluid must be excluded, which activity is safe, and which changes should send you to urgent care.
