Short answer

Swelling behind the knee without pain may come from joint fluid, a soft-tissue lump, vein problems, or a recent strain and deserves assessment if it persists.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Useful observation
Note whether the swelling changes when the knee bends or straightens.
Avoid
Do not deeply massage unexplained swelling behind the knee.
Emergency sign
Breathlessness or chest pain with leg swelling needs emergency help.

Direct answer

The absence of pain does not identify the cause. Notice whether the area feels soft or firm, changes with knee movement, or is accompanied by swelling farther down the leg.

What the swelling may mean

A fluid-filled sac connected to the knee joint can create a rounded fullness at the back of the knee. Other possibilities include swelling from irritated tendons or muscles, a harmless fatty lump, a prominent vein, or fluid collecting in the leg. A clinician may compare both knees, examine circulation and movement, and use imaging when the examination cannot distinguish these possibilities.

Clues such as recent exercise, knee stiffness, warmth, skin colour change, calf enlargement, or a lump that is growing help narrow the explanation. Avoid squeezing or repeatedly pressing a new lump.

What you can do now

Reduce activities that make the swelling more noticeable and keep the knee moving gently within a comfortable range. Resting with the leg supported may help if the lower leg also feels heavy. Do not massage deep swelling behind the knee or apply tight compression until a circulation problem has been excluded.

Arrange a routine clinical assessment if the swelling remains, returns, enlarges, or limits bending. Be ready to describe when it appeared, whether it changes through the day, and any recent injury, travel, illness, or change in activity.

When to seek urgent care

Seek urgent medical assessment if swelling develops suddenly with calf swelling, warmth, redness, tenderness, skin discolouration, or difficulty walking. Call emergency services for breathlessness, chest pain, coughing blood, fainting, or a rapidly cold, pale, numb, or weak foot. These features can accompany a blood clot or impaired circulation and should not be watched at home.

Prompt care is also appropriate when the knee becomes hot and red, fever develops, or swelling follows a significant injury and the leg cannot bear weight.

Questions for your clinician

Ask what structures could be producing the fullness, whether circulation needs checking, and whether an ultrasound or another scan would change management. You can also ask which movements are suitable, whether compression is appropriate, and what change in size, skin appearance, or leg function should trigger reassessment.

Questions people ask

Can a swelling behind the knee be present without pain?

Yes. Joint fluid, soft-tissue lumps, and vein-related swelling can occur without pain, so the feel and location alone cannot confirm the cause.

Should I exercise with swelling behind my knee if it does not hurt?

Pause strenuous or swelling-provoking exercise and use comfortable gentle movement until the cause is clearer. Sudden calf or whole-leg swelling warrants urgent assessment rather than exercise.

Will the swelling need a scan?

Not always. A clinician may request imaging when examination does not clearly show whether the swelling comes from fluid, soft tissue, or a blood vessel.
