The short answer

The pain may be sharply localised and the area can feel tender when pressed. Relative rest and gradual return to comfortable movement often help, but chest or back pain with breathing difficulty needs urgent assessment rather than self-treatment.

How this strain feels

Intercostal muscles run between the ribs and help the chest wall move during breathing. A sudden twist, forceful cough, lifting effort or direct impact can irritate their fibres. Pain typically follows a rib line from the side toward the back and becomes more noticeable when the chest expands.

Muscle tenderness and pain reproduced by a particular movement support a strain, but they do not exclude a rib, lung or other chest problem. A clinician may examine breathing, the ribs and nearby muscles when the cause is unclear.

Comfortable recovery steps

Pause movements that sharply increase pain, while continuing gentle walking and ordinary breathing as tolerated. A wrapped cool pack may calm a recent flare; later, comfortable warmth can relax guarding. Avoid tightly binding the ribs because this can limit deep breaths.

Resume lifting, reaching and rotation in small increments once they no longer provoke sharp pain. If pain persists, a physiotherapist can check trunk movement and breathing mechanics and provide graded mobility and strengthening exercises.

Signs that need medical attention

Seek urgent care for shortness of breath, crushing or spreading chest pain, faintness, coughing blood, blue lips, or severe pain after a significant impact. Fever, a new cough, a visible chest-wall deformity or pain that prevents a full breath also warrants prompt assessment.

Arrange a routine appointment if symptoms keep worsening, repeatedly return, or fail to improve with activity changes. Assessment is also sensible when the painful spot is not clearly linked to movement.

Ask whether the examination points to muscle, rib or another source; which movements are safe now; whether pain relief is appropriate with your medicines and health conditions; and what changes should trigger reassessment. Mention any injury, cough, breathing change, rash or pain spreading into the chest or abdomen.
