Azithromycin for cyst-like pimples

A painful lump beneath the skin may be nodular acne, an inflamed follicle or a boil-like abscess. These problems do not all respond to the same treatment, so taking an antibiotic without examination can delay the care actually needed.

Nodular or cystic acne presents with deep, tender inflammatory lesions and carries a greater concern for scarring than superficial whiteheads. Azithromycin can reduce susceptible bacteria and some inflammation, but it does not directly clear the blocked follicle or address a hormonal driver.

A soft lump containing pus, especially with surrounding heat and spreading redness, may instead need assessment for drainage. Spots around the mouth, bumps after shaving and rosacea-like eruptions also require different approaches. The location, texture, surrounding skin and pattern over time help distinguish them.

Do not squeeze, lance or repeatedly press the lump; this can push inflammation deeper and injure nearby skin. Cleanse gently, avoid abrasive scrubs and use non-comedogenic moisturiser and sunscreen. Arrange a dermatology appointment if deep lesions recur, leave dark marks or begin to scar.

When azithromycin is prescribed, take only the stated dose and schedule. Share any history of antibiotic allergy, liver disease, heart-rhythm problems, pregnancy and current medicines. Keep using prescribed companion acne treatments unless irritation or the clinician's instructions require a change, and attend review so oral antibiotic use is not continued without a clear benefit.

Seek prompt assessment when a lump rapidly enlarges, becomes severely painful, feels soft in the centre, drains heavily, or develops expanding redness, marked swelling, red streaks or fever. Swelling close to an eye, difficulty opening the eye, confusion or feeling faint requires urgent care.

After a dose of azithromycin, breathing difficulty, collapse, or swelling of the lips, tongue or throat is an emergency. A widespread blistering rash also needs immediate help. Report severe persistent diarrhoea, jaundice or a new irregular heartbeat promptly.

Ask whether the lump is nodular acne or an abscess, what will prevent new deep lesions, and how the plan will minimise scarring. Clarify why azithromycin was selected, which topical treatment should continue, and what finding would mean a procedure or different medicine is needed.
