Azithromycin for bacterial vaginosis

Bacterial vaginosis results from a shift in the usual balance of vaginal bacteria. Clinicians generally choose medicines that target that pattern, commonly metronidazole or clindamycin, after considering symptoms, examination findings, pregnancy, allergies, and prior treatment. Azithromycin may be prescribed when a different bacterial infection is suspected or confirmed, but it should not be borrowed or substituted for a bacterial vaginosis prescription.

Bacterial vaginosis can present with thin discharge and a noticeable odour, but itching or soreness may point toward another or an additional condition. Thrush, trichomoniasis, cervicitis, and some sexually transmitted infections can overlap in appearance while requiring different treatment. Symptoms alone cannot reliably select the right antibiotic.

Taking an unsuitable antibiotic may leave the cause untreated and can produce nausea, diarrhoea, or thrush. It can also make later assessment less clear if symptoms temporarily change. A vaginal swab or other testing may be offered when the diagnosis is uncertain, symptoms recur, or infection exposure is possible.

Take the medicine on the label exactly as directed and complete the prescribed course unless the prescriber tells you to stop. Tell them about pregnancy or breastfeeding, medicine allergies, liver problems, heart rhythm conditions, and all other medicines or supplements before starting azithromycin or an alternative antibiotic.

Avoid vaginal douching and scented internal products because they can disrupt the vaginal environment. If symptoms do not improve, return for reassessment instead of repeating leftover tablets. Testing for sexually transmitted infections may be appropriate when there is a new partner, possible exposure, pelvic pain, or bleeding after sex.

Arrange a clinical review for persistent or recurrent discharge, a new strong odour, significant itching, pain during sex, or bleeding between periods. Seek urgent care for severe lower abdominal pain, fever, fainting, heavy bleeding, or symptoms during pregnancy accompanied by pain or bleeding.

After any antibiotic, urgent help is needed for breathing difficulty, swelling of the face or throat, a blistering rash, collapse, or a fast or irregular heartbeat. Severe or persistent diarrhoea also warrants prompt medical advice.

Confirm which infection the azithromycin is intended to treat, whether a swab or sexual health test is needed, and what to do if symptoms remain after treatment. Ask about medicine interactions and whether a different formulation is more suitable during pregnancy or breastfeeding.
