Azelan for melasma: what patients should know

Melasma presents with flat, usually symmetrical brown or grey-brown patches, often on sun-exposed facial skin. Azelaic acid can reduce excess pigment formation, but it does not remove the triggers that keep pigment cells active.

Sunlight, visible light, heat, hormonal influences, and skin irritation may contribute to persistence or recurrence. That is why topical treatment is usually paired with light protection and a routine that preserves the skin barrier.

Follow the directions for the exact formulation. Apply a thin layer to clean, dry, intact skin, avoiding the eyes, lips, nostrils, and irritated areas. Begin cautiously if your skin is reactive, and use a bland moisturiser rather than adding several active products together.

Wear broad-spectrum sunscreen and use shade, a hat, and other practical light protection. Harsh scrubbing does not lift melasma and may deepen pigment through inflammation. Discuss peels, microneedling, or laser procedures with a dermatologist because poorly timed or overly aggressive treatment may provoke further discoloration.

Stop and request advice for persistent burning, swelling, cracking, blistering, or a rash spreading beyond the application area. Seek urgent help if facial swelling, widespread hives, or breathing difficulty develops.

A patch that is one-sided, raised, rapidly changing, bleeding, crusting, or markedly different from the others should be examined rather than assumed to be melasma. These features call for diagnosis, not stronger brightening treatment.

Ask whether the pattern is typical of melasma, how Azelan should be introduced around your existing products, and what level of irritation is acceptable. Mention pregnancy, breastfeeding, hormonal medicines, previous pigmentation after procedures, and any planned aesthetic treatment.
